Job Summary:
Navitas Healthcare LLC is seeking Social Worker / Case Managerfor an exciting job in New York NY.
Responsibilities:
Provide social services to clients requiring medical financial or institutional support.
Assess client needs and develop individualized social service plans.
Determine eligibility for social service programs and make appropriate referrals.
Facilitate patient placements into nursing homes and coordinate ongoing case management.
Utilize Careport for referrals and document activities in EPIC (preferred).
Conduct counseling sessions and investigatory activities to support client well-being.
Collaborate with healthcare providers social service agencies and community resources.
Qualifications:
Bachelors Degree from an accredited college or university (required).
Minimum of 3 years of related experience.
Direct experience with nursing home placements and case management.
Experience with Careport and EPIC systems (preferred).
