JOB DESCRIPTION: Care Manager Extender
SUPERVISORS TITLE: Care Manager Supervisor
WORK SCHEDULE: Office Hours Monday through Friday 9:00 a.m. to 5:00 p.m; However flexible schedule is required to accommodate members families and stakeholders which may include evening weekend hours etc. This position can be worked as remote or in-office.
SUMMARY OF POSITION RESPONSIBILITIES:
This position assists care managers in connecting members to needed services and resources while also ensuring a seamless intake experience for new members. The role requires strong organizational skills attention to detail and the ability to engage effectively with members providers and community resources.
ESSENTIAL DUTIES:
Attending daily morning check-in from 8:45am-9:00am via Google Meet.
Support care managers in coordinating and facilitating member services.
Conduct outreach engagement and follow-ups with members to ensure continuity of care.
Assist in linking members to community resources transportation and social support services.
Track referrals service authorizations and provider communications within the Electronic Health Record (EHR) system.
Participate in case conferences and interdisciplinary team meetings.
Educate members on care plans wellness initiatives and available resources.
Document all interactions and interventions per company and regulatory guidelines.
Making 15 billable contacts per week and ensuring the provision of care management services.
Submitting billable / non billable notes in EHR to reflect information above within 24 hours
Open assigned EHR charts given by TCM Intake and TCM Department
Review call list weekly and identify new members added by TCM Department

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS:
A minimum of a high school diploma or equivalency
A minimum of 2 years of experience in care management case management or a related field.
Strong understanding of tailored care management principles and the ability to coordinate care for individuals with complex health needs.
Excellent communication organizational and problem-solving skills.
Experience working with diverse populations and a commitment to cultural competence.
A valid drivers license and reliable transportation.
REQUIRED CERTIFICATION/LICENSE: N/A
PHYSICAL DEMANDS:
Regularly walk stand or stoop
occasionally lift carry push or pull
move objects weighing up to 25 pounds
regularly drive a motor vehicle
must be physically able to complete NCI-B and CPR
