QUALIFICATIONS

Education & Experience

Facilitator I

  • Bachelors degree in a related field AND
  • At least 4 years approved experience*

*approved experience is experience that qualifies the applicant for Mental Health Rehabilitation Specialist (MHRS) status in the County Childrens Mental Health System of Care

Facilitator II

  • Masters degree in Social Work Counseling Psychology with CA-BBS registration as LMFT/AMFT or LCSW/ACSW or
  • Masters degree in related field and at least 2 years or approved experience*

ESSENTIAL JOB FUNCTIONS

Position Specific

  • Coordinates and facilitates the CFT meetings Intensive Case Coordination and ensures that all team members are informed of and included in CFT meetings and follow up communication
  • Coordinates the development of Child and Family Teams (CFTs) with a focus on enhancing the involvement of natural team members and system partners
  • Guides the CFT meetings through the plan development process using accepted High Fidelity Wraparound Principles
  • Documents the plan development process by recording and distributing minutes of CFT meetings writing progress notes and updating Individualized Care Plans Safety Plans and planning documentation under the Medi-Cal model
  • Provides other case management functions as necessary securing formal resources and representing the CFT progress through the wraparound process in written documentation and verbal presentations to both internal and external partners
  • Writes clear accurate and timely progress notes consistent with each contact/visit
  • Works collaboratively with all agency programs and staff to provide support as needed
  • Attends weekly staff meetings supervision and case management staff meetings as scheduled
  • Accountable for independent time management
  • Develops and maintains positive relationships with agency and community programs
  • Utilizes and maintains Microsoft Outlook calendar with all work-related details in order to manage time effectively and share calendar information with co-workers
  • Utilizes resources to utilize behavior interventions and strategies
  • Forms positive constructive relationships with children and their families
  • Communicates effectively with all members of the Child and Family Team
  • Maintains positive relationships with consumers and customers
  • Identifies strengths skills and positive qualities in children and families
  • Identifies temporary and permanent sustainable resources and services within family and community environments
  • Assists families with transitions from resources provided or funded by Wraparound program to resources and services provided or funded by the family and community
  • Participates in the Internal Utilization Review process and other peer-review mechanisms providing feedback to colleagues accepting feedback from colleagues and promptly responding to recommendations
  • Serves as role model for the wraparound process demonstrating a commitment to family safety unconditional care family voice and choice team-driven decision-making strength-based planning and interventions flexible services and supports and community partnership and collaboration
  • Provides leadership in the overall assessment development implementation and evaluation of individualized wraparound plans
  • Prepares Individualized Family Plans and Safety Plans and coordinates each plans implementation encouraging the maximum use of natural resources and community based services and supplementing these with flexible wraparound staffing and funding
  • Provides and completes timely documentation of all delivered services including but not limited to assessment treatment planning youth and family skill building and education and care coordination.
  • Provides therapeutic interventions for skill building for example within scope weekly as determined by the CFT meeting process.
  • Clinically oversees and coordinates other services agreed upon in the CFT meeting process acting in collaboration with other team members and partners.
  • Provides other care coordination functions as necessary securing formal and informal resources
  • Participates in the Internal Utilization Review process and other peer-review mechanisms providing feedback to colleagues accepting feedback from colleagues and promptly responding to recommendations
  • Maintains accurate and timely records
  • Seeks out clinical information and advice as needed accepts constructive criticism and reasonable recommendations from supervisors
  • Mentors new Specialists
  • Ability to work flexible hours to meet the needs of the families
  • Participates in on-going training to expand and develop professional skills
  • Performs all other duties as necessary for the agency including working in other programs
  • Participate in on-going training to expand and develop professional skills
  • Perform other duties as necessary for the agency as assigned

Agency Specific

  • Performs all duties in a manner consistent with the principles and values of agency while adhering to applicable professional codes of ethics the agencys policies and procedures contractor requirements and regulatory requirements.

Employment At-Will
Employment at the Agency is terminable at-will which means that employment may be
terminated at any time without cause or reason by either the employee or the
addition the Agency may also demote layoff transfer or reassign employees at any time at its
sole discretion without cause or reason.

Salary Range:

The full salary range for this role is posted here. Offers made will fall within a portion of the range commensurate with equity factors such as experience and education.

Facilitator I: $28.00 - $40.00 Hourly DOE

Facilitator II: $70720 - $90000 Annually DOE
