Job Summary

TheCase Manager - SUDprovides case management needs assessments and care coordination to substance abuse and co-occurring clients assisting those clients in reaching self-sufficiency.

Qualifications:

  • High School Diploma or equivalent required.
  • Completed MHACBO minimum training and experience requirements for CADC I or higher.

Primary Responsibilities:

  • Collaborates with the client on a needs assessment to identify areas of focus
  • Assists with recovery plan development and implementation coordinating with the primary counselor and other key staff
  • Investigates resources and provides referrals to appropriate community agencies
  • Integrates client supports (family social community) to help remove barriers for clients
  • Coordinates referrals within the ASAM level of care to residential facilities to affect a smooth transition between services
  • Documents in a timely and appropriate manner each intervention and coordinates information with the clients support team
  • Attends staff meetings and development trainings as scheduled
  • Participates in clinical supervision including QA review of documentation and personal goal tracking highlighted in yearly evaluation
  • Adheres to agency policies and procedures including Ethics Statement
