Job Summary

TheCase Manager - SUDprovides case management needs assessments and care coordination to substance abuse and co-occurring clients assisting those clients in reaching self-sufficiency.

Qualifications:

High School Diploma or equivalent required.
Completed MHACBO minimum training and experience requirements for CADC I or higher.

Primary Responsibilities:

Collaborates with the client on a needs assessment to identify areas of focus
Assists with recovery plan development and implementation coordinating with the primary counselor and other key staff
Investigates resources and provides referrals to appropriate community agencies
Integrates client supports (family social community) to help remove barriers for clients
Coordinates referrals within the ASAM level of care to residential facilities to affect a smooth transition between services
Documents in a timely and appropriate manner each intervention and coordinates information with the clients support team
Attends staff meetings and development trainings as scheduled
Participates in clinical supervision including QA review of documentation and personal goal tracking highlighted in yearly evaluation
Adheres to agency policies and procedures including Ethics Statement
