Job Summary

PURPOSE

Reporting to the Manager of Health Services the Treatment and Aftercare Coordinator is responsible for supporting Gwichin Participants of the Gwichin Comprehensive Land Claim Agreement (GCLCA) as they navigate the continuum of care related to substance use mental health and wellness recovery. This position plays a critical role in coordinating access to treatment programs and providing culturally informed aftercare support that aligns with the Gwichin Tribal Councils (GTC) wellness priorities mandates and annual workplans. The coordinator works directly with clients families and external service providers to develop individualized care plans facilitate referrals and ensure continuity of care before during and after treatment. With a strong understanding of trauma-informed practice and cultural safety the coordinator supports community-based recovery approaches that reflect Gwichin values traditions and healing practices. This position contributes to the development of wellness strategies provides advocacy within regional and territorial systems and informs programming that improves long-term health outcomes for Gwichin Participants.

RESPONSIBILITIES

  • Provide intake assessment and referral support for individuals seeking treatment for substance use and mental health concerns.
  • Develop and maintain individualized aftercare plans in collaboration with clients families and service providers.
  • Support Participants throughout the treatment journey including pre-treatment preparation transportation logistics and post-treatment reintegration.
  • Liaise with territorial and national treatment facilities and ensure clients are supported in accessing appropriate culturally relevant services.
  • Advocate for Gwichin Participants within healthcare and wellness systems to ensure equitable access to supports.
  • Deliver culturally grounded aftercare support including participation in healing gatherings land-based wellness programs and traditional practices.
  • Maintain confidential client files and track program outcomes using GTC-approved reporting systems.
  • Participate in community engagement education and awareness initiatives related to addiction recovery and wellness.
  • Contribute to strategic planning and service improvement initiatives within the Health and Wellness Department.
  • Coordinate client access to land-based healing programs cultural camps and traditional counselling services.
  • Monitor clients progress post-treatment and adjust aftercare plans as needed to reflect changing goals or circumstances.
  • Support the reintegration of clients into their home communities with a focus on relapse prevention cultural connection and community resources.
  • Collaborate with justice education housing and social service providers to create wraparound supports for clients.
  • Maintain a comprehensive directory of treatment centers detox programs and community wellness resources.
  • Track and analyze service delivery data and contribute to program evaluations and reporting requirements.
  • Participate in crisis intervention and emergency response planning as it relates to client wellness.
  • Promote harm reduction principles and provide information on safe substance use practices where appropriate.
  • Provide wellness outreach during community events gatherings or emergencies in coordination with other GTC staff.
  • Support GTCs advocacy efforts for improved access to culturally appropriate treatment and aftercare services in the region.
  • Attend training and professional development sessions to maintain knowledge of best practices in treatment coordination and Indigenous wellness.

QUALIFICATIONS

  • Diploma or degree in Social Work Addictions Counselling Mental Health or a related field (equivalencies with lived experience and training will be considered).
  • Minimum 2 years of experience in wellness addictions or mental health support services.
  • Demonstrated knowledge of culturally safe and trauma-informed care practices.
  • Familiarity with treatment and recovery systems in the Northwest Territories and nationally.
  • Knowledge of and ability to use MS Office suite of applications (Word Excel PowerPoint and Outlook) Operating Systems Email and Internet programs
  • Experience working with Indigenous clients and communities; knowledge of Gwichin culture is a strong asset.
  • Excellent interpersonal organizational and communication skills.
  • Ability to maintain confidentiality and manage sensitive information appropriately.
  • Valid Class 5 Drivers License and ability to travel as required.
  • Ability to provide a clean Criminal Record Check.
