Position Summary

Navitas Healthcare LLC is seeking Licensed Practical Nurse (LPN) Long Term Carefor an exciting Travel or Local job in Montour Falls NY.

Key Responsibilities

Deliver high-quality patient care during evening shifts.
Collaborate with interdisciplinary teams to support continuity of care.
Document all patient care activities accurately in the EMR system.
Assist with patient admissions discharges and transfers.
Maintain a safe clean and organized environment.
Respond promptly to patient needs and communicate effectively with patients and families.
Follow infection control and safety procedures consistently.

Qualifications

Minimum 2 years of relevant clinical or healthcare experience.
Active and unrestricted LPN license in the state of New York.
Ability to work evening shifts with alternating weekends and holidays.
Strong communication critical thinking and time management skills.
Proficiency with electronic medical records (EMR) systems.
Ability to work both independently and as part of a team in a fast-paced environment.
