Job Summary:

This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with the interdisciplinary team. Strong communication and collaboration skills are essential.

- Minimum two years of hospital home health or hospice care experience.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.

Special Considerations:
- Role may involve working in a fast-paced environment and providing care to patients with varying levels of acuity.
- Requires use of a personal vehicle for field work.
- Return to office (RTO) policy must be noted at time of submission.
