Job Summary:
This role involves providing direct patient care in a sub-acute care center specializing in patients with ventilator and tracheostomy dependencies. The Nurse Assistant will work collaboratively with a team of nurses CNAs and respiratory therapists to provide care and support to patients requiring sub-acute rehabilitation home care or long-term care services. Experience in long-term care or skilled nursing is required.

Responsibilities:
- Providing direct patient care including assistance with activities of daily living (ADLs).
- Monitoring patients vital signs documenting observations and reporting changes in condition to licensed nurses.
- Assisting with patient hygiene mobility and positioning.
- Collecting specimens and assisting with medication administration as directed by a licensed nurse.
- Maintaining a clean and safe environment for patients.
- Performing basic nursing tasks as delegated by a licensed nurse (e.g. suctioning pulse oximetry).
- Collaborating with the interdisciplinary team to ensure comprehensive patient care.
- Performing clerical duties such as answering phones and documenting patient information.
- Floating to other units within the scope of practice.
Required Skills & Certifications:
- Current Certified Nursing Assistant (CNA) license.
- Minimum of one year of experience in long-term care or skilled nursing.
- Basic Life Support (BLS) certification.
- Ability to work with patients requiring tracheostomy and ventilator support.
Preferred Skills & Certifications:
- Experience in sub-acute care settings.
- Experience with patients requiring tracheostomy and ventilator support.
Special Considerations:
- Must be comfortable working with patients who have tracheostomies and ventilators.
- Floating between units within the sub-acute care center is required.
- Block scheduling may be possible per manager discretion.
- Facility allows for one call-off per two-week pay period.
- May be required to float to a nearby acute care hospital.
- Must have reliable transportation within a 50-mile radius.
Scheduling:
- Shift type and scheduling details will be determined by the manager.
- The possibility of block scheduling will be considered on a case-by-case basis.
