Job Summary:
This role is for an Occupational Therapist providing skilled home health services in the East Portland OR and Clackamas County areas. The OT will manage a caseload of up to six patients per day providing one-on-one care and adhering to a 30-productivity-point standard. The position requires a minimum of one year of skilled home health experience and proficiency in Oasis charting. A valid Oregon license and BCLS certification are mandatory.

Responsibilities:
- Provide one-on-one occupational therapy services to home health patients.
- Manage a caseload of up to six patients daily meeting a 30-productivity-point standard.
- Conduct patient assessments and develop individualized treatment plans.
- Administer occupational therapy interventions.
- Maintain accurate and timely documentation in Oasis and EPIC.
- Collaborate with the interdisciplinary team.
- Manage travel between patient locations within a 60-mile radius.
Required Skills & Certifications:
- Current Oregon Occupational Therapy license.
- Minimum one year of skilled home health experience.
- Basic Life Support (BLS) certification.
- Proficiency in Oasis electronic health record system.
- Valid drivers license car insurance and reliable transportation.
- COVID-19 vaccination (primary series and one booster).
Preferred Skills & Certifications:
- Experience with EPIC electronic health record system.
Special Considerations:
- 8-hour day shifts; no 4x10 or 3x12 schedules accommodated.
- Up to one shift per pay period may be cancelled by the facility.
- Business casual attire; scrubs allowed but not required.
- Occasional travel up to 60 miles (60-90 minute drive time) may be required.
- No on-call requirements.
- No weekend requirements.
Scheduling:
- 8-hour day shifts.
- Shift assignments are determined by the facility.
- No block scheduling accommodated.
