Community Health Navigator St. Bernardine Medical Center
As a Community Health Navigator at St. Bernardine Medical Center you will be a key advocate for patients and community members guiding individualsparticularly those from vulnerable and uninsured populationsthrough the healthcare system and connecting them with essential services. This role plays a crucial part in improving access to care reducing disparities and promoting long-term wellness through education resource coordination and outreach.
Key Responsibilities
- Support patients and community members by navigating medical social service and community-based resources.
- Build trust-based relationships to encourage healthcare access and continuity of care.
- Provide culturally and linguistically appropriate health education and self-navigation support.
- Facilitate connections to Federally Qualified Health Centers (FQHCs) low-cost clinics and outreach programs.
- Collaborate with hospital staff and community partners to coordinate care and promote public health initiatives.
- Uphold the mission values and strategic goals of St. Bernardine Medical Center in every interaction.

Qualifications
- Associate Degree preferable in a healthcare related field preferred
- Experience in a healthcare setting working with patients healthcare services medical groups and payers is required.
- Ability to use Google Office Suite Microsoft Office Suite (specifically Word Excel Outlook and Access).
- Demonstrate ability to navigate the internet and Windows-based software applications.
- Able to provide culturally appropriate service.
- Ability to build effective relationships with patients staff community agencies and community members.
- Knowledge of healthcare payors medical groups and services.
- Understanding of medical terminology
- Good written and verbal communication skills. Bilingual Spanish speaker with ability to read and write Spanish preferred/.
- Valid California Drivers License in good standing required.
