You would be joining a mighty team of support and administrative staff clinicians physicians nurses and others in our efforts to improve the health and well-being of our community.
Were hiring for a full-time Clinical Care Specialist to join our Community Response/MEIS Team 41 Montebello Rd. Pueblo CO 81001.
Work schedule: 12-hour shifts
10 am 10 pm varied Monday through Saturday
Report to Supervisor of team
What Youll Be Doing
As the Clinical Care Specialist for the Mobile Early Intervention Services (MEIS) Team you will provide mobile community-based outreach and support to individuals in Pueblo County who are at risk of experiencing a behavioral health crisis. The MEIS Team operates on a referral basis typically responding within 24 hours of receiving a referral.
Key Responsibilities:
- Provide brief support regular check-ins and early intervention services in community-based settings to help stabilize individuals and prevent escalation into behavioral health crises.
- Teach and reinforce coping strategies symptom management and daily living skills during times of stress trauma or life transitions to promote resilience and improve day-to-day functioning.
- Identify stressors coping mechanisms and natural supports to aid in crisis prevention coping plan development and re-engagement of individuals who have become disconnected from services.
- Offer hands-on assistance and motivational support to help clients engage in ongoing behavioral health treatment and recovery.
- Provide education on mental health and substance use support the development of problem-solving skills and increase understanding among individuals and families.
- Link clients to medical behavioral health and community resources.
- Assis with scheduling attending appointments and arranging transportation as needed.
- Act as a liaison and coordinate care with internal Health Solutions programs and external partners to ensure continuity of care and integrated support.
- Collaborate with individuals families and support systems in treatment planning resource navigation and promoting safety stability and recovery.
- Utilize evidence-based practices and comply with Health Solutions policies state standards and clinical guidelines.
- Maintain timely accurate documentation and adhere to all data reporting requirements.
- Attend clinical consultations team meetings and staffings as required.
- This is a driving-intensive position that requires safe and reliable operation of a company vehicle throughout your shift.
- Perform other job related duties assigned by the director or their designee.
Physical requirements include the following:
- Requires full range of body motion.
- Manual and finger dexterity and eye-hand coordination.
- Requires standing walking and staying stationary for prolonged periods.
- Occasionally lifts and carries items up to 25 pounds.
- Requires exposure to communicable diseases and bodily fluids.
- Requires working under stress in emergencies.
- Requires a normal range of hearing and eyesight to record data electronically and communicate with patients.
- Frequently operatesa computer and other office equipment to record health data.
- Constantly communicates with clients and others.
- Frequently drives between facilities or for other assigned duties.

Qualifications
What Were Looking ForThe Must-Haves
- Bachelors degree in social work counseling psychology or a closely related field (required).
- Experience working with substance use disorders (SUD) mental health disorders and co-occurring disorders (required).
- Strong computer literacy and ability to use electronic health records and other clinical systems.
- Valid Colorado drivers license
- Knowledge of behavioral health systems trauma-informed care harm reduction and motivational interviewing.
- Strong interpersonal organizational and communication skills.
- Comfortable working in field and fast-paced environments including homes streets and public spaces and with law enforcement.
What Wed Like to See in YouThe Nice-to-Haves
- Spanish language skills
- Colorado CAC II III or LAC
- Experience in community-based crisis intervention case management or working with law enforcement is strongly preferred.
