This role is for a Physical Therapist providing skilled home health services to patients in the West Portland OR area including Newberg Dundee and Willamina. The therapist will manage a caseload of up to six patients daily providing one-on-one care and utilizing electronic charting systems. A minimum of one year of skilled home health experience is required.

The therapist will manage a caseload of up to six patients daily providing one-on-one care and utilizing electronic charting systems. Location: Portland Oregon United States
Responsibilities:
- Provide one-on-one physical therapy services to patients in their homes.
- Manage a caseload of up to six patients per day.
- Conduct patient assessments and develop treatment plans.
- Administer physical therapy interventions.
- Maintain accurate and timely documentation in electronic health records (EPIC and Oasis).
- Collaborate with other members of the healthcare team.
- Manage travel between patient locations within a 60-mile radius.
- Manage a caseload of up to six patients per day.

Required Skills & Certifications:
- One year of skilled home health experience.
- Current Physical Therapist license in Oregon.
- Basic Life Support (BLS) certification.
- Proficiency in Oasis documentation system.
- Valid drivers license car insurance and reliable transportation.
- Ability to manage a caseload of up to six patients per day.

Special Considerations:
- 8-hour day shifts; no 4x10s or 3x12s accommodated.
- No weekend requirements.
- No on-call responsibilities.
- No floating between units.
- Block scheduling is not accommodated.
- Up to one shift per pay period may be cancelled by the facility.
- Mileage reimbursement at the IRS standard rate.
- Business casual attire; scrubs allowed but not required.
