Your Role as a Pediatric Registered Nurse Case Manager:

As aCase Manageryoull play a vital role incoordinating and overseeingthe care of pediatric patients while serving as a key advocate for families. Your responsibilities will include:

• Patient Assessments & Care Planning Conduct comprehensive assessments develop individualized care plans and adjust treatments as needed.

• Care Coordination & Advocacy Collaborate with physicians therapists and caregivers to ensure seamless high-quality patient care.

• Family Education & Support Teach families about treatment plans disease management and best practices for home care.

• Clinical Nursing Care Provide direct care including medication administration wound care tracheostomy/ventilator management and emergency interventions.

• Monitoring & Documentation Track patient progress recognize changes in condition and ensure accurate documentation within the clients record.

• Leadership & Team Collaboration Work effectively within a multidisciplinary team demonstrating flexibility and problem-solving skills in various healthcare situations.

• Compliance & Quality Assurance Uphold Elara Carings mission philosophy and policies to maintain the highest standards of care.

What You Need to Succeed:

• Education: High School Diploma or GED

• Graduate of an accredited Certificate Diploma Associate or Baccalaureate School of Nursing.

• Experience At least 1 year of RN experience with Pediatric experience

• Physical Requirements - Ability to sit stand bend lift and move intermittently with the capability to lift 50-100 lbs.

• Licensure Active and unrestricted RN license in the state of practice.

• Transportation Reliable vehicle valid drivers license and current auto insurance (per state laws).

• Skills & Attributes Strong clinical judgment problem-solving abilities and the ability to communicate effectively with families and healthcare teams.
