Your new role

As an Intensive Case Manager you will deliver the Journey to Social Inclusion (J2SI) program providing long-term personalised support to people experiencing chronic homelessness. Working with a small caseload youll use a trauma-informed team-based approach to help clients access housing build life skills and foster independence. J2SI is delivered by a consortium of community organisations and funded by the Victorian Government.

Your skills and experience
  • Qualification relevant to the delivery of community services e.g. Social Work Psychology Occupational Therapy or equivalent. Candidates with Diploma qualifications and outstanding relevant experience may be considered.
  • Valid drivers licence to drive in Australia.
  • Demonstrated experience in case management
  • Proven ability to restore relationships with people experiencing vulnerabilities and history of complexities in areas such as adult homelessness alcohol and other drugs mental health disability and family violence.
  • Ability to work effectively in an outreach capacity
  • Sound knowledge of various theoretical frameworks and strategies applicable to working with clients with complex needs.
