TITLE: Licensed Independent Certified Social Worker Consultant
SUPERVISOR: Adult Day Health Director
Compensation: DOE $100-$120/hour
QUALIFICATIONS:
- Must be a LICSW in Massachusetts. Masters degree in social work or human services from an accredited college and at least one years recent experience working with adults in a professional capacity.
- Must possess sensitivity to the needs of low-income older adults. Able to work cooperatively as a multi-disciplinary team member
POSITION SUMMARY:
The Social Worker Consultant will serve as a team advisor guiding the Care Plans Assessments and participants needs within the Adult Day Health setting and their home environment. This Social Worker will meet with the Case Managers once a week to coordinate lead and guide all the Social Services and Documentation needs of the Case Managers.
A once-a-week face-to-face meeting is necessary for this position. This includes a discussion with the participants and sometimes with their family members.
They will guide the development implementation and evaluation of social service plans for each assigned participant assuring excellent care monitoring and follow-through by coordinating and overseeing services done by the case managers with other agencies/caregivers and continuously evaluating all service and care plans to see that needed services are received.
RESPONSIBILITIES AND DUTIES:
Primary Job Functions
- Meet with the case managers in the Adult Day Health Facility and develop a documented workflow process.
- Develop update and teach Case Managers the social service plans (admission and progress notes and discharge plans)
- Work with the Director of Nursing and Nurses to develop and update the clinical needs of the participants.
- Help the case managers if needed to coordinate services with other agencies.
- Train or lead the Case Managers for individual and group counseling with participants and their caregivers as needed.
- Participate in family meetings and case conferences.
- Develop Behavioral Plans for participant behavioral issues.
- Assist Case Managers in determining clinical eligibility for Adult Day Health services
- Ensure Case Managers complete all assessments and documentation in a timely matter to meet MassHealth requirements
