22.02 - 24.00 USD Per Hour

Working Location:

Merced CA

Working Hours/Shift:

Monday-Friday 830AM to 5PM

This position is eligible for our Commuter Benefit Program for:

This position is eligible for commuter benefits for those who travel more than 20 miles one way from their residence to their work location.
Eligible employees approved for the Commuter Benefit Program will receive $200 per month for regular full-time employees and $100 per month for regular part-time employees.

What you bring to SBHG:

Associate degree in a social science field required plus three (3) years experience providing support to individuals living with mental illness or providing support to individuals in a behavioral health setting or experience performing non-professional healthcare or social services.
Bachelors degree in a social science field preferred plus one (1) year experience providing support to individuals living with mental illness or providing support to individuals in a behavioral health setting or experience performing non-professional healthcare or social services..
Valid California Drivers License required

How you will make a difference:

Identifies housing resources and developing relationships with community agencies.
Assists clients in completing facility consent forms upon admission.
Actively participates in treatment team meetings and is responsible for working with
other disciplines to develop a coordinated treatment plan for assigned clients.
Provides consultation as needed with families and the Case Manager on issues regarding vocation education and housing.
Assists clients and families in utilizing available resources to meet client and family
needs.
Facilitates resident council community meeting and/or groups as instructed.
Transports clients to appointments to provide direct linkage to resources.

Division/Program Overview:

24/7 inpatient voluntary program
16-bed facility for adults (ages 18-59)
Rehabilitation/recovery services including substance use rehabilitation services
Pre-vocational or vocational counseling
Medication evaluation and support services
