Your role:
The Case Manager will oversee the care team of specialist professionals in delivering case management for high risk high needs young people.
The position objectives are:
- Case Manager is to provide leadership and coordination of the care team for each young person placed in the Eastern region Residential Care program.
- Provide support to young people in the program and lead the multi-disciplinary team to develop and implement trauma informed plans for each young person.
To Be Successful in this Position You Will Need:
- Tertiary qualifications preferably in social work / psychology youth work or equivalent which includes diploma level qualifications.
- Commitment to providing quality care and have experience in leading multi-disciplinary teams.

- You get a salary starting at around $100776 plus super
- Salary packaging is available up to $15900 plus an entertainment card.
- SCHCADS award level 5.
- Full time 38 hrs weekly Fixed-Term until 30th June 2026.
