Physical Therapist (PT) Long Term Care/SNF - Travel Position
Location: Trenton TN
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s Days
Pay: $1955/week
Job Description:
We are seeking a dedicated and compassionate Physical Therapist (PT) to join a skilled nursing and long-term care facility in Trenton TN. The PT will evaluate plan and implement individualized treatment programs to help patients restore mobility improve strength and maximize independence. The ideal candidate is patient-focused detail-oriented and experienced in geriatric and rehabilitative care settings.
Key Responsibilities:
- Conduct comprehensive patient evaluations to assess physical function mobility and rehabilitation needs.
- Develop and implement individualized treatment plans in accordance with physician orders and facility protocols.
- Provide therapeutic exercises manual therapy gait training and functional mobility interventions.
- Educate patients families and caregivers on therapy goals home exercise programs and safe mobility techniques.
- Collaborate with interdisciplinary teams including nursing occupational therapy and speech therapy to ensure coordinated care.
- Document patient progress treatment plans and outcomes accurately and timely.
- Ensure compliance with facility policies professional standards and state/federal regulations.
- Support patients in achieving maximum functional independence and quality of life.
Qualifications:
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Tennessee (or compact license).
- Minimum of 1 year of experience as a Physical Therapist preferably in long-term care or skilled nursing settings.
- Current Basic Life Support (BLS) certification.
- COVID vaccine and recent TB test
- Strong clinical reasoning communication and organizational skills.
- Compassionate approach to patient care with experience supporting geriatric populations.
