POSITION SUMMARY
A Clinical Navigator oversees the planning scheduling and implementation of day-to-day clinical activities for navigated patients and facility partners. There are administrative duties such as handling calls and correspondence liaising with key stakeholders developing strategies to improve operations and processes and resolving issues should there be any.
CLINICAL NAVIGATOR RESPONSIBILITIES
- Central point of contact for all navigated patients medical staff and families.
- Assist with referrals to outside resources and care coordination to include Home Health orders Hospice orders DME orders etc.
- Management of labs imaging for AL/IL facilities.
- Handle all medication refill requests from pharmacies and facilities including prior authorizations.
- Complete necessary paperwork including LTC insurance paperwork Incompetency letters Handicap placards etc.
- Read labs and x-rays and communicate results with patients and families.
- Other duties as assigned

- Position is a Hybrid position partially remote and partially in office
QUALIFICATIONS
- Certified Medical Assistant is required
- Requires 2-5 years of experience in healthcare fields.
