SUMMARY:
The Assistant Clinic Manager and Care Coordinator is responsible for supporting the Clinic Managers with daily clinic operations and the care coordination of patients with chronic illnesses or who are high-utilizers of health care. This position is an integral part of the Patient-Centered Medical Home care team and works collaboratively with others to support optimal patient outcomes.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:
Education: High School diploma or GED equivalent.
Experience: One year of clinical setting experience required. One year experience in a non-profit clinic/community service setting or related experience and/or training; or equivalent combination of education and experience. Coordination of clinic flow experience preferred.
Knowledge & Skills: Knowledge of non-profit clinic flow coordination preferred. Knowledge of medical terminology Medi-Cal and other payer sources preferred. Must be able to learn quickly in a fast paced environment. Must possess interpersonal skills to interact diplomatically with staff and difficult clients. Must be able to work with a very diverse group of patients in a non-judgmental manner. Computers skills and working knowledge in the use of standard software such as Windows Word Excel and PowerPoint.
Must possess strong verbal and organizational skills. Bilingual in English and Spanish preferred.
Physical Demands: May be required to lift objects weighing up to 25 pounds. Sitting in a normal seated position for extended periods of time. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s) for example using a keyboard. Communication skills using the spoken word. Ability to see within normal parameters. Ability to hear within normal range. Ability to move about.

Salary Range: $74000 - $78000 annually
