Suburban Hospital is a 228-bed Level II Trauma Center and a community-based hospital in Bethesda Maryland. What you will do:
The Care Coordination Registered Nurse (Case Manager) evaluates timeliness and appropriateness of patient care and coordinates discharge or transition planning for a smooth progression to post-hospital care. Utilizing a collaborative process in conjunction with Care Management Coordinator Social Workers and other health care team members the Case manager will assess plan implement and evaluate the options and services required to meet an individuals health and health related needs. The Care Manager serves as an advocate for the patient family and organization to promote quality cost effective outcomes. Active participation in program development and process improvement efforts is an expectation of the role of the Care Manager.

Salary Range: Minimum 53.02/hour - Maximum 53.02/ will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
