Overview:
Case Management for Intercambios Puerto Rico executing SAMHSAs Grant for the Benefit of Homeless Individuals. Position requires applicant to be able to conduct outreach and work in community setting across the eastern section of Puerto Rico (including San Juan Metropolitan Area).
Responsibilities:
- Provide services to individuals (and families) who are experiencing homelessness substance use and/or co-occurring mental health disorders directly in the community.
- Responsible for all documentation necessary to create the participants record: receive review and file all required documents.
- Provide evidence base practices according to the needs of the target population among these: SBIRT Motivational Interview Relapse Prevention Harm Reduction and document interventions.
- Screen individuals (mostly in the community) for recruitment to the GBHI Project.
- Complete intake and follow up GPRA.
- Maintain an updated register of participants and provide the necessary follow-ups within the established timeframe.
- Carry out the processes in compliance with the rules procedures and protocols of the organization.
- Assess needs of program participants create treatment plans and review treatment plans.
- Coordinate and assist in participant transportation to services.
- Link program participants to care and support services via referrals and follow up.
- Link participants to housing via referrals and follow up.
- Referred and coordinate services with internal and external programs for participants in need.
- Document and update the participant record according to the established timeframe.
- Provide follow up services to program participants according to the established timeframe.
- Represent project and agency in different forums.
- Coordinate with other team members for participant services or support.
- Visit organizations and services providers to create collaboration relations for the benefit of the GBHI Project.
- Driving the official or rented vehicle when necessary to complete project tasks.
- Prepare monthly reports.
- Bachelors in social work Behavioral Sciences or equivalent
- At least five years of experience with providing case management services to people that have experienced homelessness substance use and/or co-occurring disorders.
- Knowledge about MAT/HIV/AIDS Hepatitis-C and STDs in the Puerto Rican context
- Knowledge and/or experience with Evidence Based Practices / Interventions
- Highly motivated to work with people who experience homelessness substance use and/or co-occurring disorders in an outreach setting.
- Proficient and efficient with multiple computer software programs
- Meticulous at maintaining records and confidentiality protocols.
- Good written and verbal communications skills (English a plus)
- Outreach and participant navigation experience
- Able to work closely with others in outreach team and office setting (team player)
- Up to date drivers license
