Location: Liverpool Office (L7)
Salary: 26780 per annum up to 2600 on-call payments
Bonus: Up to 300 new starter bonus 300 annual bonus 300 refer-a-friend bonus
Hours: Full-time 37.5 hours per week (rota basis including weekends)
Contract: Permanent
Requirement: Must have a valid UK driving licence and access to a vehicle
About the Role
We are looking for a caring compassionate and highly organised Care Assessor to join our award-winning organisation. As a Care Assessor youll work alongside families health and social care professionals and community partners to co-design personalised care plans that meet individual outcomes.

Key Responsibilities
Care Planning & Reviews
- Co-design personalised Care Plans with service users and families.
- Complete reviews and risk assessments ensuring they remain accurate and responsive to changing needs.
- Introduce Care Workers to customers supporting smooth transitions.
Communication & Collaboration
- Liaise with Social Workers GPs District Nurses and other professionals.
- Keep families informed and involved in care decisions.
- Support Care Workers to fully understand and deliver individual outcomes.
Prioritisation & Administration
- Respond promptly to new referrals and hospital discharges.
- Maintain accurate records care notes and electronic systems.
- Identify risks and escalate appropriately.
Safeguarding & Mentoring
- Ensure safe practices are embedded into care planning and delivery.
- Mentor and support Care Workers to deliver high-quality care.
- Take part in the on-call rota providing support to staff and customers as needed.
What Were Looking For
- Experience in adult social care ideally in assessment care planning or mentoring.
- Excellent communication skills both written and verbal.
- Strong organisational skills and the ability to prioritise under pressure.
- A values-driven approach caring compassionate and committed.
- A full UK driving licence and access to a vehicle.
