Job Summary

Role Responsibilities

Conduct initial and ongoing assessments to evaluate patient mobility functional limitations and rehab needs.
Develop and implement individualized evidence-based physical therapy plans: therapeutic exercises gait/balance training manual therapy wound care assistive device training modalities (e.g. moist packs ultrasound etc.)
Monitor and evaluate patient progress adjust plans as needed.
Educate patients families and caregivers on home programs safety and preventive measures.
Collaborate with interdisciplinary team and participate in care/discharge planning; attend care conferences/utilization reviews/in-service
Document thoroughly and timely in compliance with facility state and federal standards.
Supervise PTAs therapy aides and students as appropriate.
Discharge patients when outcomes are met and ensure follow-up or referrals as needed

Qualifications & Requirements

License: Active Missouri PT license (Compact accepted in some cases)
Experience: Minimum 1 year in rehab or long-term care (LTC) required; new grads with sufficient clinical experience may be considered Certifications: BLS/CPR (AHA) required
