Position Summary
The Staff Pharmacist provides comprehensive pharmacy services to patients with focus in cardiology (inpatient including intensive care and outpatient) at Toronto General Hospital with cross-coverage to other areas as required.
Duties
- Review and dispense all doctors medication orders for appropriateness
- Identify resolve and prevent drug therapy problems with the patient and health care team
- Monitor drug therapy for effectiveness toxicity and adherence to evidence based medicine/guidelines
- Conduct medication reconciliation
- Follow-up on medication incidents and promote safe medication practices on the unit
- Respond to drug information questions
- Patient education
- Provide direct and/or functional supervision of technician staff
- Facilitate and support clinical teaching and research activities within the department while exemplifying UHNs values
Qualifications :
- Possess a Bachelors of Science Degree in Pharmacy Doctor of Pharmacy degree or equivalent
- Certificate of competency from the Pharmacy Examination Board of Canada
- Active registration with the Ontario College of Pharmacists (Part A)
- Hospital Pharmacy residency an asset; hospital pharmacy practice experience with patient-centered care
- Excellent verbal and written communication skills
- Excellent interpersonal skills
- Excellent organizational and time management skills
- Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
