Position Summary:
Delivers high quality cost-effective pharmaceutical care to hematology/oncology and infusion patient populations in the clinic setting by participating in activities such as drug selection drug information dosing monitoring preparation outcomes management and patient education
Reports to: Pharmacy Manager
FLSA: Non-exempt
ESSENTIAL FUNCTIONS:
- Evaluates prescribers medication orders with respect to overall patient therapy and consults with
physicians and other health providers on the selection dosages interactions and side effects of
medications.
- Verifies and/or enters patient medication orders accurately into pharmacy information system and
ensures proper charging/crediting of products on patient accounts.
- Prepares labels and dispenses hazardous and non-hazardous medications accurately and efficiently
for patients in accordance with physicians orders and established policies and procedures.
- Provides medication education to providers patients and/or caregivers as needed.
- Supervises the activities of pharmacy technicians to ensure timely and accurate medication delivery
and compliance with all professional standards and organizational policies and procedures.
- Assists in inventory purchasing stocking and quality control of drugs and products utilized by
pharmacy.
- Contributes to assessing maintaining documenting and overseeing pharmacy cleanroom
environment in accordance with USP 797 USP 800 and all facility policies and procedures.
- Assists with development and implementation of evidence-based medication guidelines and protocols.
- Performs clinical review entry and modification of oncology and infusion treatment plans within the
clinical information system.
JOB REQUIREMENTS:
- Bachelor of Science from accredited School of Pharmacy - Required
- Doctor of Pharmacy from accredited School of Pharmacy - Preferred
Required Skills
Requires critical thinking skills organization decisive judgement and the ability to work with minimal supervision in a stressful environment. Must be able to communicate effectively with patients and other healthcare providers.
Required Licenses
Pharmacist
Active Michigan Pharmacist License - Required
Active Michigan Controlled Substances License - Required
Specialty Certification in Oncology (BCOP HOPA core competency) - Preferred
Minimum Work Experience
At least one year in a hospital or oncology pharmacy. - Required
Three years in oncology or infusion practice - Preferred
