- Serve as the primary contact for new client inquiries; conduct in-home assessments to evaluate care needs.
- Develop personalized care plans and match clients with appropriate caregivers.
- Provide ongoing support to clients and families ensuring satisfaction with services.
- Collaborate with the scheduling and care teams to ensure seamless service delivery.
- Identify and cultivate relationships with referral sources including hospitals rehab centers senior living communities case managers social workers and physicians.
- Attend community events health fairs and professional networking meetings to promote agency services.
- Track outreach efforts and maintain a pipeline of potential client leads.
- Develop and implement strategies to increase brand awareness and grow the client base.
- Bachelors degree in Healthcare Social Work Business or related field (or equivalent experience).
- Minimum 2 years of experience in home care senior services or healthcare sales/marketing.
- Excellent interpersonal communication and presentation skills.
- Self-starter with a results-driven mindset.
- Reliable transportation and willingness to travel locally.
