PRIMARY FUNCTION The Behavioral Health Case Manager supports the care coordination needs of patients experiencing a mental health or substance use disorder. Works with patients to identify social barriers to recovery by developing and implementing a person-centered care coordination plan. Collaborates closely with members of the WHC care team as well as other community service organizations. PRINCIPLE DUTIES
1. Assess patient needs and develop patient centered care coordination plan that addresses social barriers to mental health and substance use disorder recovery. Supports patients in accessing needed resources through close follow up and care coordination. 2. Provide health education and proactively engage patients in activities to reduce the impact of mental health and substance use disorder on health outcomes. 3. Serves as an internal reference for resources available to support mental health and substance use disorder recovery. JOB REQUIREMENTS:
Minimum Education & Experience: High School Diploma or Degree in Social Work preferred. At least two years experience in a patient care setting. Lived experience in mental health or substance use disorder recovery preferred. Required License:Basic Life Support Certification (American Heart Association certified) Qualifications: Successful candidates will be skilled in identifying social barriers to recovery enthusiastically engaging with patients and be adept at quickly developing working relationships patient action plans and follow-ups to support the social needs and health goals of patients. Candidates must be proficient in time management able to multi-task competing priorities and be critical thinkers. Ideal candidates will have strong verbal and written communication skills timely documentation and maintain a high level of professionalism. TYPICAL WORKING CONDITIONS Office environment with frequent contact with staff and the public. Work may include transporting patients to appointments when needed in company vehicle. ACCOUNTABILITY Reports to Patient Support Services Manager. SALARY
