The Department of Economic Security Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated individual to join our team as a Quality Assurance Incident Management Specialist. DDD provides individuals with developmental disabilities and their families services and supports that are flexible high quality and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.

Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement and cannot be employed or subcontracted by a DDD Qualified Vendor.

Essential Duties and Responsibilities include but are not limited to:
Coordination of investigations conducting investigations and completing investigation reports. Compile reports and send via interoffice mail to each support coordinator.
Entry of incident report into the Incident Management Systems (IMS) notification of key personnel (including Support Coordination Quality Assurance and Region Administration) of these incidents and the tracking of this communication and follow up. Research each incident report through the Client Application of FOCUS and the District Employee Directory in order to ensure member vendor and internal distribution information is correct.
Conducts training and technical assistance for qualified vendors/service providers and division staff. Drives to and from licensed settings to complete health and safety reviews.
Answers the incident reporting phone line and documents the calls via a call log ensures entry into IMS. Follows up with the afterhours emergency staff and the afterhours administrator to ensure information is accurate and thorough for Quality Assurance review and possible entry into IMS. Follows up with vendors for written reports on incidents that were verbal notifications.
Receives incident reports via fax email and verbal notification. Performs initial review for seriousness and priority before compiling for Quality Assurance Review.

Investigation and fact-finding methods.
Human Dynamics and organizational systems.
Routine letter and memo composition.


Skills In:
Effective interviewing Oral and written communication.
Maintaining and updating databases files ensuring conformity in day-to-day operations with department and division procedures.
Working with material of a technical and/or confidential nature dealing with the public using appropriate tact and resourcefulness.
Compile organize analyze and interpret data and information into report formats.


Ability to:
Write summarize and produce difficult and complex reports concisely.
Use various computer software applications (Word Excel Database systems etc.) prioritize and meet time lines.
Assist with timely projects reports memos etc. Screen and route correspondence and request follow up.
Learn interpret and apply Division programs policies and procedures.
Learn interpret and apply Department procedures needed to perform discretionary tasks.

If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business then the following requirements apply: Drivers License Requirements. Any employee operating a motor vehicle on State business must possess and retain a current valid class-appropriate drivers license complete all required training and successfully pass all necessary drivers license record checks. The license must be current unexpired and neither revoked nor suspended. Employees who drive on state business are subject to Drivers license record checks must maintain acceptable driving records and must complete all required driver training. An employee who operated a personally owned vehicle on state business shall maintain the statutorily required liability insurance (see Arizona Administrative Code (A.A.C.) (R.11).
Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults.
Candidates for this position shall be subject to a search of both the Department of Child Safety Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agencys ability to reasonably accommodate any restrictions.
