Job Description

ho We Are:
The FMT is composed of a select group of compassionate and skillful individuals who share a common goal of striving to provide quality care to a unique population. Our program is designed to divert individuals involved in the criminal justice system from entering state mental health treatment facilities by managing mental health needs in a community setting through wraparound services. We utilize a collaborative treatment approach aimed at addressing the individualized needs of each client.
Who We Serve:
Adults in Brevard County with a chronic persistent mental illness; a pending felony charge; who is adjudged incompetent to proceed (ITP) not guilty by reason of insanity (NGI) or deemed at-risk for decompensation without treatment intervention.
What We Provide:
The FMT provides an array of to include but are not limited to: medication management; individual therapy; case management; assistance with entitlement benefits; community and jail-based competency restoration training; assistance in obtaining stable housing; referring clients to community resources; and linking clients to appropriate aftercare services at the completion of our program.
Responsibilities:
Attend court hearings with the individual as needed
Prepare written status reports as requested by the Court probation officers or community providers
Monitor compliance with the conditional release plan
Assess safety and quality of the current living environment
Provide alternative housing options for clients in need of housing
Meet with housing providers to negotiate the cost of living
Maintain an up to date housing resource list
Aid in the identification of housing options for clients newly admitted
Assist clients with obtaining and maintaining benefits
Network with community partners to develop relationships to successfully transition individuals from the team
Identify additional resources to address unique individual needs
Maintain a caseload of up to 15 clients
Provide and/or coordinate services including transportation to appointments when needed
Must be comfortable working in a community jail Court and office setting
Must be capable of utilizing designated electronic medical records system and keeping documentation as required by the standards of the program
Be able to work independently as well as part of a team
Requirements:
  • A minimum of a Bachelors Degree in behavioral science or be credentialed as a Certified Recovery Peer Specialist.
  • A minimum of one year of work experience with adults with serious mental illnesses.
  • Possess a valid drivers license and reliable transportation.
