We are seeking highly motivated and compassionate PACE PT Physical Therapists fulltime for our Empath Manasota Team in Bradenton. As part of our Physical Therapy Team you will be part of a dynamic Clinical Team who ensures that our patients return and maintain their optimum level of physical functioning. The Physical Therapist:
- Under the supervision of the Rehab Manager provides physical therapy services to participants of Stratum LIFE/PACE.
- Provides clinical physical therapy evaluation and treatment for program participants on acute restorative or maintenance levels as needed at Day Center ALF Nursing Home or in-home setting.
- Integrates the physical therapy treatment plan into the Interdisciplinary Team comprehensive plan of care.
- Assists in the ordering inventory distribution and maintenance of durable medical equipment for participants.
Requirements:
Education: Be a graduate of a physical therapy curriculum. Current State of Florida Physical Therapy license required.
Minimum: Bachelors Preferred: Masters
Minimum one year of experience working with the frail or elderly population
Intermediate proficiency in MS Word Excel and PowerPoint MS outlook.
Be a graduate of a physical therapy curriculum approved by one of the following: The American Physical Therapy Association; The committee on Allied Health Education and Accreditation of the American Medical Association; The council on Medical Education of the American Medical Association and the American Physical therapy Association required. Other organizations approved by the Secretary.
Valid CPR certification from the American Red Cross or American Heart Association.
Empath Manasota PACE (Program of All-Inclusive Care for the Elderly) is a nonprofit provider of comprehensive health care and support services to Manatee County seniors with chronic health conditions.
