POSITION SUMMARY
In keeping with the mission statement of the hospital and the patient service units the Care Facilitator is to provide clinical and practice support to the nursing staff. This position will facilitate interdepartmental communication and will be instrumental to the management staff on call throughout the weekend for issues related to staffing and patient flow.
JOB DESCRIPTION
- Co-ordination of patient movement as the need arises facilitating in a timely manner the efficient functioning of the clinical unit with attention to assessing and directing care and flow of patients.
- Provide just-in-time clinical /practice support to nurses.
- Facilitate staff mobilization.
- Provide on-going clinical/technical education that is PSU specific.
- Responsible for policy dissemination.
- Fosters the provision of family-centered care by supporting and assisting nursing staff with patient care as deemed necessary and facilitating the advancement of evidence-based practices.
- Act as a resource person for unit staff supporting and promoting efforts leading to the accomplishment of goals and individual objectives.
- Assists staff members in carrying out the nursing process as appropriate.
- Works cooperatively with the Clinical Manager Advanced Practice Nurse Nurse Educator and Operations Director to maintain continuity of patient care.
- Provides leadership in quality practice.
- Monitors and ensures that unit standards of care are maintained through direct observation of care revision of charting and evaluation of equipment.
- Facilitates and initiates Quality Improvement activities and participates in unit quality management programs.
QUALIFICATIONS

- BScN and a minimum of seven (7) years RN experience in a pediatric intensive care clinical environment including a minimum of two (2) years experience in a nursing leadership role and a minimum of two (2) years preceptor OR other appropriate combination of education and experience along with the demonstrated knowledge skills and abilities to perform the duties of the position may be considered in lieu of stated Education and Experience (Essential)
- Current registration with the College of Nurses of Ontario (Essential)

RSUM DES FONCTIONS
Conformment lnonc de mission de lHpital et des units de service aux patients le Facilitateur ou la Facilitatrice de soins fournira un soutien clinique et pratique au personnel infirmier. Il ou elle facilite la communication entre les services. Son rle est crucial pour aider le personnel dadministration qui est de garde pendant les fins de semaine rsoudre des problmes de dotation et de dplacement des patients.
DESCRIPTION DU TRAVAIL
- Coordonner le dplacement des patients selon les besoins faciliter en temps opportun le fonctionnement efficace de la clinique en se concentrant sur lvaluation et la direction des soins et sur le dplacement des patients.
- Fournir en temps opportun un soutien clinique et pratique aux infirmiers et infirmires.
- Faciliter la mobilisation du personnel.
- Offrir continuellement de lducation clinique et technique propre la USP.
- Se charger de la diffusion des politiques.
- Favoriser la prestation de soins axs sur la famille en soutenant et en aidant les infirmiers et infirmires prodiguer les soins aux patients tel que ncessaire et en promouvant les pratiques fondes sur des donnes probantes.
- Assumer le rle de personne-ressource pour le personnel de USP des soins intensifs en soutenant et en promouvant les activits qui contribuent latteinte des buts et des objectifs individuels.
- Aider les infirmires et infirmiers accomplir le processus infirmier tel que ncessaire.
- Cooprer avec le ou la gestionnaire clinique avec linfirmier ou linfirmire en pratique avance avec linfirmire enseignante ou linfirmier enseignant ainsi quavec la directrice des oprations afin de maintenir la continuit des soins aux patients.
- Fournir un leadership favorisant la qualit des pratiques.
- Surveiller les normes de soins que prodigue lUnit en observant directement les soins en examinant la consignation des donnes dans les dossiers et en valuant lquipement.
- Faciliter et lancer des activits damlioration de la qualit et participer au programme dassurance de la qualit de lUnit.
QUALIFICATIONS
- Vrification des antcdents en vue dun travail auprs de personnes vulnrables (Essentiel)
