Job Description

Traditional Home Health & Hospice
Join our team of professionals and make a difference in someones life.

Job Title: RN/LPN Hospice Nurse
Shift: Full-time part-time and per diem schedules available. Our office hours are Monday thru Friday 8:30am to 5:00pm.
Job Location: New Milford PA and surrounding areas.
Our nurses see patients in the most ideal setting - where the patient resides!
At Traditional we do our best to keep each nurses case load within a sensible geographic territory.

Job responsibilities include but not limited to the following:

  • Admit patients to Traditional Home Health Care (Hospice) and initiate comprehensive assessments. (RNs only)
  • Develop and implement individualized plans of care in collaboration with physicians nursing supervisors patients and family members.
  • Provide direct patient care including clinical treatments personal care medication administration and patient/family education in accordance with physician orders.
  • Continuously assess patient condition and respond to changes by consulting with physicians and adjusting the care plan as necessary.
  • Maintain clear and consistent communication with physicians nursing supervisors therapists and other members of the care team.
  • Accurately and promptly document all patient interactions care provided medication changes case conferences and OASIS time points as applicable.
  • Act as a resource and advocate for patients by informing them of available services and making appropriate referrals to community resources.
Requirements:
  • Current license as an RN or LPN in Pennsylvania
  • Must have current drivers license
  • Ability to travel to case assignments
  • Excellent assessment skills documentation skills and ability to meet deadlines
  • Ability to work independently with minimal supervision and manage time effectively
  • Strong interpersonal skills.
  • Demonstrates good verbal and written communication and organizational skills
  • Acute care and/or home health experience is preferred.
  • Having experience with hospice patients or in the hospice environment is a plus.
  • Understanding of Hospice philosophy and principles of death and dying.
