You will be updated with latest job alerts via emailCommunity Housing Partners is seeking candidates for a Senior Services Coordinator (SSC) in HUDs grant-funded multifamily housing program with a focus on senior and disabled populations. We seek a highly motivated individual with a strong work ethic and dedication to serving underserved populations through the coordination and facilitation of services. The SSC is responsible for the implementation and coordination of service delivery to individuals within an assigned geographic area encompassing Pulaski VA and the surrounding counties.

What Were Looking For:
Resident Services establishes partnerships and linkages with other service providers to develop and implement program activities focusing on three content areas: Community Programs Housing Stability and Senior Services. Our staff work with our residents to increase their quality of life by assisting residents in reaching their individual goals and increasing their self-sufficiency skills. This is accomplished by conducting individual and/or community assessments to determine services needed and then connecting residents with or delivering resources or services based on those needs. Experience working with senior populations and persons with disabilities case management benefits and entitlements and in-home or facility-based services is important for this position.
Excellent networking skills are essential as this position actively builds and maintains partnerships with local resources such as community agencies schools business leaders and communities of faith to ensure clients have access to needed support and services. Compassion empathy and a strong desire to help disadvantaged populations are essential as are creativity active listening skills and the ability to analyze results to determine success. Successful candidates will be able to effectively demonstrate strong problem-solving customer service and communication skills.
A Day in the Life of:
A Senior Services Coordinator (SSC) works with residents who are aged 55 and older and non-elderly individuals with disabilities to help them age in place in healthy and stable homes.
Did you know that the U.S. population is aging and the number of folks 65 and older will increase by 39% by 2040 and that individuals aged 85 and older will grow by 56% Additionally 80% of older adults have at least one chronic health condition and one in four older adults experience behavioral health problems such as depression anxiety or substance abuse. This position requires you to have the ability to interact with older adults to identify areas of need and assist in the following categories:
Resident Engagement (mental/emotional support encouraging socialization identifying barriers)
Programming to address barriers increase socialization and build rapport.
Community linkage (food pantry/distribution clothing distribution financial support medication delivery primary/specialty care benefits and entitlements)
To be successful as an SSC one must understand that the ability to age in place for as long as possible is vital to the health and welfare of our Seniors. The best SSCs have superb organizational skills intrinsic motivation for helping and experience working with older adults who may present physical and behavioral health conditions.
Basic Qualifications:
High School Diploma or equivalent plus 2 years of experience working in the human services/social services field; OR
Bachelors degree in the human services/social services field with knowledge of and training in elderly and disability services.
Demonstrated working knowledge of local services for the elderly and people with disabilities.
Experience working directly with elderly populations or persons with disabilities preferred.
