Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. Strong communication and collaboration skills are essential. A minimum of two years of experience is required.

Location: Rochester New York United States
Responsibilities:
- Patient assessments and monitoring.
- Medication administration.
- Treatment planning and implementation.
- Collaboration with interdisciplinary team (physicians other nurses therapists etc.).
- Case management.
- Hospice care (if applicable).
- Documentation of patient care.
- Maintaining patient records.
- Medication administration.
- Treatment planning and implementation.

Required Skills & Certifications:
- Clinical assessment skills.
- Medication administration skills.
- Treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of hospital home health or hospice care experience.
- Drivers license car insurance and personal automobile for field work.
- Medication administration skills.
- Treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of hospital home health or hospice care experience.

Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
