Job Summary:
This Registered Nurse position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (hospital home health or hospice).
- Maintaining accurate patient records and documentation.

- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.

- Drivers license car insurance and personal vehicle for field work.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- Weekend work is required (Fri 0700-1930 Sat-Sun 0730-2000).
- On-call requirements are to be determined.
- Floating between units may be required.
- Block scheduling may be possible per manager discretion.
- RTO (Return to Office) must be included at the time of submission.
Scheduling:
- Friday: 7:00 AM - 7:30 PM
- Saturday & Sunday: 7:30 AM - 8:00 PM
- Specific shift assignments and scheduling details will be determined.
