Job Description
Overview
Responsibilities
Collaborates with patients care team members to ensure patient is connected to resources that support overall health and helps patients establish connection to primary care provider and appointments as appropriate.
Collaborates with other team members to build a library of community and healthcare resources.
Assists patients navigate the healthcare system helping the patient identify and address and eliminate barriers to accessing care.
Actively participates in team meetings to provide input and feedback on patient needs and successful strategies for addressing these needs.
Collects and helps analyze data from Cone Health and public data sources to identify and close gaps in care and address health disparities.
Screens patients for health-related social needs and connect patients appropriately to available resources while helping patients explore their own motivation to change identify barriers set goals and make progress toward improved health.
Qualifications
Preferred: Completion of North Carolina Community Health Worker Standardized Core Competency Training Program (NC CHW SCCT)
Preferred: Experience working in healthcare setting and experience health coaching within historically marginalized populations.
