Qualifications:
Required:
- Masters degree in education (i.e. Moderate Disabilities 5-12) or a related vocational/transition field.
- DESE licensure in education (i.e. Moderate Disabilities 5-12) and/or similar certification in a related vocational/transition field (i.e. CESP ACRES VRC et cetera).
- A valid MA drivers license and safe driving record.
Preferred:
- Experience working with high school and transition age young adults with exceptional needs.
- DESE Transition Specialist Endorsement.
Duties and Responsibilities:
- Will review Transition Planning Forms and collaborate with the IEP Team to create IEP objectives calculated to meet the students postsecondary goals.
- Will coordinate the schools community work program and develop employment opportunities for students in the community.
- Will supervise job coaching staff including one lead job coach and other job coaches.
- Will provide consultation to classroom teachers to ensure community trips are centered on students IEP and TPF goals.
- Will plan and maintain a transition curriculum to be implemented through the classrooms with the support of classroom teachers and instructional aides/job coaches.
- Will perform vocational and transition assessments for specific students (i.e. three year reevaluations as requested by district) to inform the IEP and team process.
- Will coordinate with districts state agencies and local service providers in regards to the status of Chapter 688 referrals and to ensure seamless transitions to adult services and supports.
- Will coordinate student participation in the Massachusetts Inclusive Concurrent Enrollment Initiative programs.
- Will work with school administration to collect Indicator 14 data on post-school outcomes.
- Will meet regularly with the Director of Transition Services of the SEEM Collaborative to provide updates gather feedback and establish departmental priorities for the program.
- Will participate in Team meetings (three year re-evaluations annuals progress/update) as needed.
- Will be responsible for leading departmental meetings and trainings and will participate in school leadership meetings trainings and professional development as needed.
- Will strive to create and maintain a safe nurturing environment for every student that aligns with the goals of the program.
- Must be able to lift pull or push 40 pounds or more and have the ability to move quickly from standing to floor and floor to standing.
- Must be willing to work with students who exhibit behavioral challenges.
- Employment is contingent upon passing a SAFIS fingerprint and CORI screening.
- Other duties as assigned by the Program Principal.
Evaluation:
Will be evaluated on an ongoing basis by the Program Principal and will receive at least one formal evaluation annually by the Program Principal.
