Job Title: HCC Nurse
Work Location: Thunder Bay Ontario and rotation to all KO remote communities
Employment Type: Part-Time/Full-Time
SUMMARY
Reporting to the HCC Nurse Manager HCC Nurse develops coordinates and implements in-home nursing services works with a team of Personal Support Workers (PSWs) and other health care professionals. This individual will directly provide in-home client-centered nursing services to HCC clients in various KO communities. The HCC Nurse will fulfill responsibilities in accordance with the standards of practice and guidance of the College of Nurses of Ontario.
RESPONSIBILITIES
- Develops coordinates and implements in-home nursing services under the direction of the HCC Nurse Manager and in conjunction with KO community Nurse in Charge and community Health Director.
- Works with a team of Personal Support Workers (PSWs) unregulated healthcare providers and other healthcare professionals in the program.
- Maintains a relationship with the medical professionals in the community the Chief and Council and other stakeholders.
- Maintain current skills through attendance at workshops in-service and staff meetings.
- Assists the HCC Nurse Manager with preparing reports for the KO Director of Nursing (DON) and annual year end reports for Health Canada.
- Performs other duties requested by HCC Nurse Manager KO Health Director or DON.
- Charting using the Community Electronic Medical Record (cEMR) Mustimuhw.
KNOWLEDGE SKILLS AND ABILITIES
- A Registered Nurse (RN)/ Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
- Minimum 2 years experience delivering nursing care in a home or community setting.
- Advanced Foot Care certified; Mental Health nursing an asset.
- Knowledge on culture and tradition of remote First Nation communities.
- Excellent verbal and written communication skills. Ability to speak Ojibway Oji-Cree or Cree an asset.
- Ability to prepare and submit reports.
- Basic computer skills and willing to learn a new cEMR system.
- Awareness of working with unregulated Health Workers; Transfer/Delegation of Skills processes and requirements.
- Schedule: Rotational basis 2-4 weeks in communities. To be discussed
Hours of Work and Travel Expectation
- 8:30 AM-4:30 PM-Monday to Friday inclusive
- Overtime maybe required as needed
- Travel to remote First Nation communities on a rotational basis
