Physical Therapist - Home Health (Full-Time) $72-$80/hr

Core Responsibilities:
- Deliver evidence-based physical therapy services in patients homes
- Perform comprehensive physical assessments and develop personalized treatment plans
- Execute therapeutic interventions focused on improving mobility strength and function
- Provide patient and family education on wellness safety and rehabilitation
- Document care using electronic health records (EHR) systems
- Coordinate with interdisciplinary healthcare teams
- Maintain compliance with Medicare HIPAA and state regulations
Required Qualifications:
- Masters degree in Physical Therapy (DPT preferred)
- Current state Physical Therapy license
- Minimum 1 year of home health experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong interpersonal and communication skills
- Proficiency in EHR documentation
