JOB SUMMARY
Serves as a Long-Term Care and Dementia Care Navigator supporting individuals with Alzheimers Disease and Related Dementias (ADRD) and their caregivers. Cross-trained to perform Maryland Access Point (MAP) functions including providing information referral and options counseling. Collaborates with the Adult Evaluation and Review Services (AERS) team to conduct in-home evaluations and assess cognitive decline. Supports future integration with Senior Care case management by participating in joint home visits and conducting dementia-related assessments. Assists with care planning and service coordination for older adults with cognitive impairment.

MINIMUM QUALIFICATIONS
Education:A bachelors degree from an accredited college or university.
AND
Experience:Two years of paid and/or volunteer experience social work human services and/or health care.
OR
Equivalency Notes: Have an equivalent combination of education and experience.

Background Check

Probation
