JOB SUMMARY
Serves as a Long-Term Care and Dementia Care Navigator supporting individuals with Alzheimers Disease and Related Dementias (ADRD) and their caregivers. Cross-trained to perform Maryland Access Point (MAP) functions including providing information referral and options counseling. Collaborates with the Adult Evaluation and Review Services (AERS) team to conduct in-home evaluations and assess cognitive decline. Supports future integration with Senior Care case management by participating in joint home visits and conducting dementia-related assessments. Assists with care planning and service coordination for older adults with cognitive impairment.
ESSENTIAL FUNCTIONS
Supervision
- Supervises a team of community health workers (CHWs) including evaluating their performance providing technical assistance and discipline.
- Coordinates teams schedules and provide daily remote supervision and troubleshooting.
- Oversees the CHWs activities generally and in the field as needed.
- Tracks daily and weekly progress for CHWs including cases contacted successfully referred and successfully connected to services resources and benefits.
- Conducts quality assurance by listening in on calls and provides feedback to CHWs.
- Approves time and leave requests.
Community Education and Outreach
- Connect clients and families with appropriate community-based services and resources including those offered through MAP the Alzheimers Association and health care providers.
- Organizes and conducts educational activities such as participation by Maryland Access Point in service and health fairs town halls and other public events.
- Makes presentations on services benefits and resources available to older adults and persons with disabilities including services and resources for persons with ADRD and their caregivers.
- Assists in developing Maryland Access Point brochures web site submissions and social media postings.
- May supervise/lead community outreach work in designated Baltimore City neighborhoods.
Training
- Assists in developing staff training programs including curricula presentations and supplemental materials.
- Provides training and coaching to CHWs on services to persons with ADRD and their caregivers.
- Provides orientation program to new staff including direction on customer service record keeping protocols and use of technology.
Data Management and Reporting
- Oversees data collection processes by MAP Team members and manages quality assurance reviews of data records.
- Prepares reports for internal use by the Baltimore City Health Department and for submission to funding agencies.
- Submissions may include portions of annual plans and grant applications.
- Analyzes trends in services as documented by data collected by MAP operation.
MINIMUM QUALIFICATIONS
Education:A bachelors degree from an accredited college or university.
AND
Experience:Two years of paid and/or volunteer experience social work human services and/or health care.
OR
Equivalency Notes: Have an equivalent combination of education and experience.
KNOWLEDGE SKILLS AND ABILITIES
- One year of supervisory/management experience.
- Knowledge of Microsoft Office Suite products.
- Familiarity with call center technology and applications.
- Excellent interpersonal skills required and ability to interact professionally with culturally diverse individuals during a time of crisis and distress.
- Excellent organizational and communication skills.
Preferred Knowledge Skills and Abilities:
- Experience working with older adults and/or persons with disabilities.
- Second or multiple languages a plus.
- Familiarity with the geography and the neighborhoods of Baltimore City.

Background Check

Probation
All persons including current City employees selected for this position must complete a 6-month mandatory probation.
