Description

The opportunity:
We are looking for multiple Care Managers to join our supportive and professional multidisciplinary teams in a full time of part time capacity.

Responsibilities include but are not limited to;

  • Providing quality case management services by embracing contemporary practices and compliance with Aged Care Quality standards
  • Provide support and advice to home care package consumers and their carers to support them to achieve their goals and remain living at home and in their community in accordance with the Home Care Package Operational Manual
  • Developing and documenting consumer directed care plans and assessment with regard to their needs and preferences
  • Provide ongoing care management for Home Care Packages (HCP) for the consumer/carer conduct regular reviews of their care needs and goals amend care plans and supports and services as appropriate
  • Liaise and advocate with other relevant service providers regarding individual care needs.
  • $500 sign on bonus - T&Cs apply


Skills And Experiences

Our ideal candidate:

  • Minimum of 3 years experience in Home Care Packages program and/or Tertiary qualification in Social Work Allied Health Nursing Care Management or equivalent
  • Extensive experience working with older people and/or with Home Care Packages consumers
  • Demonstrated experience in Consumer Directed Care (CDC) choices and preferences to consumers
  • Knowledge of aged care related specialist services i.e. Parkinsons and Dementia support Services would be an advantage
  • Knowledge of budget management and fees and costs associated
