Our Senior Doc MOBILE Primary Care Team is expanding in your community and were looking for a Nurse Practitioner or Physician Assistant with a heart for serving seniors. This is the perfect blend of working independently while having the backing of a close-knit clinical team and support staff.

Your Role:
As a Senior Doc Provider youll care for seniors where they feel most comfortabletheir homes assisted living residences and care communities across the San Fernando Valley. Youll be responsible for:
- Conducting patient visits with care efficiency and clinical excellence
- Managing chronic conditions and coordinating care across settings
- Collaborating with a supportive clinical and administrative team
- Providing consistent compassionate communication to patients and families
