Overview:
We are seeking aFamily Nurse Practitioner (FNP) to join a respected healthcare team in New Haven CT. This is a fulltime permanent opportunity focused on delivering highquality patientcentered care across the lifespan in a collaborative environment.

Key Responsibilities:
Provide primary and preventive care to patients of all ages
Conduct physical exams diagnose conditions and prescribe treatment plans
Educate patients on health maintenance disease prevention and wellness
Collaborate with physicians and interdisciplinary care teams
Maintain accurate and timely patient documentation
Requirements:
Masters degree in Nursing (MSN)
Licensed (or eligible) as a Nurse Practitioner in the state of Connecticut
Certification as a Family Nurse Practitioner (FNP)
Excellent communication and clinical decisionmaking skills
