Position: NORC Supervisor
Department: Aging Health and Wellness Services
Reports to: Executive Director & Deputy Director of Aging Health and Wellness Services
Hours: FullTime
Salary:$58500 $63000

PositionSummary:
The NORC Program Supervisor is responsible for providing staff and intern supervision supportive counseling and geriatric care management services to residents age 60 of the Franklin Plaza Apartments in East Harlem.
Responsibilities:
- Provide daytoday supervision of direct service NORC program staff interns and volunteers.
- Deliver supportive counseling and case assistance to older adult residents addressing issues such as mental health social isolation financial insecurity and health care navigation.
- Conduct comprehensive geriatric assessments and develop individualized care plans in collaboration with clients families and healthcare providers.
- Coordinate services and referrals with external agencies healthcare providers and community resources.
- Organize and support community programming that promotes health wellness and social engagement among senior residents.
- Monitor client progress and maintain accurate confidential case records in accordance with agency and funding requirements.
- Input data into the DFTA database and maintain comprehensive electronic and paper case files including intakes care plans case progress notes event participation and units of service provided as well as any important documents.
- Ensure program goals and outcomes are met including tracking and reporting data for internal and external stakeholders.
- Participate in community outreach and relationshipbuilding within the Franklin Plaza Apartments and broader East Harlem community.
- Assist and help lead special events for NORC participants such as parties concerts and trips.
- Assist with grant reporting budgeting and evaluation as needed.
- Additional duties as assigned by supervisor.
Qualifications:
- Masters degree in Social Work (MSW) Gerontology Counseling or a related field required.
- LMSW or LCSW preferred.
- Minimum 2 years of experience in geriatric social work care management or related human services field.
- Prior supervisory experience strongly preferred.
- Knowledge of agingrelated resources benefits and entitlements in New York City.
- Experience working with NYC Aging/ a NORC programs is a plus.
- Strong organizational communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
- Bilingual (English/Spanish or English/Chinese (Mandarin/Cantonese) a plus.
