About the role
This flexible 36hour weekly role permits a combination of office and remote work. Officebased work is also an option with free parking available at our Colindale office.

As part of our supportive team you will work with individuals to facilitate quick safe hospital discharges. Once these individuals return to their community you will support them in leading the life they aspire to live placing their wellbeing at the heart of everything we do.

When required you will conduct complex individual assessments and safeguarding enquiries using the appropriate tools and techniques. This will ensure clear relevant proportionate and appropriate strategies are established to meet their identified outcomes.

You will work in partnership with adults their families carers advocates health colleagues and external organisations. Your goal is to provide appropriate professional social work support for adults with care and support needs transitioning from hospital to community living.

You may conduct Mental Capacity Act assessments in accordance with statutory guidance ensuring adults are provided with the opportunity to demonstrate capacity where possible.
