Responsibilities:
- Provide oversight and mentorship to our clinicians in the greater Greenville North Carolina area
- Conduct clinical case reviews share tools information and guidelines as they relate to quality of care
- Represent our facility in engaging establishing and maintaining relationships within the community
- Provide clear communication with clinicians facility leaders and staff regarding care and services for enrollees
- Collaborate with leadership to review care coordination initiatives
- Develop intervention plans and share ideas about network management issue
- Work alongside pharmacists on medication management
- Complete Medicare required visits
- Complete acute follow up and discharge visits as necessary
- Implement an individualized plan of care for patients goals
- Make recommendations on transitional care DME home health services or outpatient services after short stay
- Facilitate and assist leadership in quality improvement initiatives and risk management
- Timely response in chart completions and pharmacy recommendations
*Clinicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM) geriatric primary internal (IM) adult postacute (PAC) home health hospice palliative and hospital medicine (HM).
