Job Title: Compassionate Therapist Make a Difference in the Lives of Older Adults!
Location: California (Occasional local travel)
Schedule: MondayFriday

About the Role:
As a Therapist you will play an essential role in providing highquality behavioral health care to seniors ensuring they receive the support and attention they need. This is your chance to make a real difference in the lives of older adults while collaborating with an incredible care team. Youll be responsible for conducting behavioral health assessments creating tailored care plans and guiding the interdisciplinary team (IDT) on behavioral health best practices to ensure exceptional outcomes.
Key Responsibilities:
Provide behavioral health diagnosis therapy and care planning for a wide range of mental health challenges.
Collaborate with the IDT to educate and assist in behavioral health assessments goal setting and interventions.
Ensure seamless integration of behavioral health needs into participant care plans.
Build strong relationships with community behavioral health specialists.
Follow up with participants admitted to inpatient psychiatric facilities supporting them through postdischarge recovery.
Assess and diagnose participants with unclear behavioral health conditions to establish effective care plans.
Train staff on critical behavioral health topics.
Support the behavioral health needs of participants and contribute to the success of the organization by taking on additional responsibilities as necessary.
Qualifications
Required:
Masters degree or higher.
3 years of experience as a Therapist with at least 2 years of experience working with:
Adult populations.
Elderly individuals.
Active California license/certification:
Licensed Mental Health Counselor (LMHC)
Licensed Professional Clinical Counselor (LPCC)
Licensed Clinical Social Worker (LCSW)
Licensed Independent Clinical Social Worker (LICSW)
Preferred:
1 years of PACE (Program of AllInclusive Care for the Elderly) experience.
Bilingual skills are a plus.
