Clinical Care Coordinator

About the Role:

As a Clinical Care Coordinator youll lead our clinical and care teams to ensure residents receive exceptional care while nurturing an understanding of their unique identities and experiences. Youll also play a pivotal role in our homes clinical governance and may manage specialized clinical portfolios such as education palliative care and infection control.

Key Responsibilities:

  • Ensure comprehensive clinical and specialized nursing care is provided documented and tailored to each residents needs by our clinical staff.
  • Prioritize residents and their families in decisionmaking considering their preferences independence dignity and identified carerelated risks.
  • Oversee and conduct all scheduled ANACC reappraisals including mandatory and voluntary assessments as required.
  • Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting care planning and addressing patient concerns promptly.
  • Conduct routine clinical assessments to promptly address identified gaps in care.

About you:

  • Currently registered as a Nurse with AHPRA with no restrictions.
  • Demonstrated senior management experience in the aged care sector.
  • Indepth knowledge of the aged care industry with a keen eye for emerging trends and best practices.
  • Proficiency in managing human resource matters complemented by strong negotiation skills.
  • Skilled in utilizing various information technology tools and software including Word and Excel with a quick ability to adapt to new skills as required.
