SUMMARY
Under the direction of the PCP and Practice Manager the Resource Nurse is responsible for ensuring that patients receive timely high quality and efficient health care and support services within and outside of the Patient Centered Medical Home. The Resource Nurse works closely and collegially with physicians associate providers clinical and administrative staff and collaborates to enhance the delivery of patient care services along the continuum of care. Works closely with the nurse care manager and assists in the management of patients. Promotes patient satisfaction by interacting with patients in a professional friendly manner consistent with their needs.
RESPONSIBILITIES
Under the direction of the Primary Care Provider and in collaboration with the PCMH Care Manager:
Direct Patient Care
Assists with phone triage for complex urgent calls.
Assists Care Team when office emergencies arise:
Monitors patient and provides support to patient/family
Provides hand off to EMS and ED (in collaboration with Provider)
Assists the Care Team to remain in flow:
When necessary and if MA unavailable:
Administers injections medications and provides complex wound care
Assists with minor surgery office laboratory and other tests
Instructs patients regarding preparation for various tests and procedures.
Communicates and interprets providers clinical information and instructions to patients.
Provides patient education/coaching and gives handoff to Care Manager if appropriate.
